Registration Form
When
registration begins the registration form will
be available here in two formats:
1. PDF (.pdf) format:
Anyone can download and open using the
near-universal free Adobe Acrobat reader. The
form can be saved and printed for manual
data entry. Click on
link that will be provided later.
2. Microsoft
Excel (.xls) format: Anyone can download. It can only be opened if you have the free
Microsoft Excel viewer or if you have the full
Excel program installed. The free Excel viewer will
let you view and print the spreadsheet but not
edit it. The full Excel program will let you save,
view, print, and edit (i.e., fill in the form on
your computer with the “math” done by the
spreadsheet’s formulas). Click on link that will
